approach to dyspepsia according to helicobacter pylori status in iran

چکیده
helicobacter pylorus (hp) is a common infection in iran. inappropriate laboratory tests for its diagnosis such as serology and irrational drug prescription for hp treatment in patients with non-ulcer dyspepsia is a drastic obstacle in our country. treating every patient with dyspepsia without peptic ulcer or precancerous lesions, not only results in high economic burden on health system, but also causes antimicrobial resistance and undesirable clinical outcomes. the iranian society of gastroenterology and hepatology discussed this topic in a meeting and suggested preparing a guideline for approach to dyspepsia in iran. then, the literatures were reviewed and the published data in our area were searched and a practical guideline was developed. according to the present evidences, we suggest hp treatment only for patients with peptic ulcer, infected gastric cancer patients and their first degree relatives and also for malt lymphoma. we suggest standardized urea breath test, endoscopic biopsy or stool antigen test for diagnosis of hp infection and its eradication. serology is not a suitable diagnostic method for evaluation of hp infection even prior to the treatment. because of ineffectiveness, we don't suggest hp eradication treatment for non-ulcer dyspepsia.

It is clear that non-ulcer (or functional) dyspepsia is a heterogeneous syndrome that includes a subset of patients with unrecognized gastroesophageal reflux. Patient heterogeneity combined with inadequate study methodology has led to enormous confusion in interpreting the relationship between Helicobacter pylori and non-ulcer dyspepsia. The possibility that H. pylori is associated with gastroe...

متن کاملRole of Helicobacter pylori in functional dyspepsia.
The aetiology of dyspepsia is unknown in the majority of patients. Helicobacter pylori (H pylori) is the cause in a subset of patients. A non invasive test to assess the presence of H pylori is recommended in the management of patients under the age of 50 presenting to a family practitioner with dyspepsia. A urea breath test or a stool antigen test are the most reliable non invasive tests. Erad...
متن کاملHelicobacter pylori—Associated Dyspepsia in Paediatrics
Helicobacter pylori ubiquitously infects the human gastric mucosa since time immemorial, predictably before the man’s diaspora out of East Africa around 58,000 years ago [1]. Colonization may have been somehow beneficial for human carriers, allowing the coevolution of this gram-negative bacterium and its host over the centuries. Yet, at least nowadays [2], this may not be a peaceful association...
